Headache or cephalalgia is  a pain in the head with the pain being above the eyes or the ears, behind the head (occipital), or in the back of the upper neck.Headache is a non specific symptom,  which means that it has many possible causes, including stress., loss of sleep, common cold, head injury., brain tumor etc.

Headache is a very common symptom experienced by almost everyone. Headache itself can be a diseases or it can be due to some other underlying pathology.
Causes of headache :According to IHS (International Headache Society)there are more than 200 types of headache and so are the causes. Headache can be due to simple causes like infection or sinusitis(inflammation of sinus ) etc to more serious causes like tumor in the brain etc. Headaches can occur suddenly or they may be chronic in nature. Headache can be triggered by missing a meal or stress or use of perfume or increased physical activity, noise etc. 

Type of Headache

Headache is broadly classified into primary headache and secondary headache.

Primary Headache : Primary headaches are benign, recurrent headaches not caused by underlying disease or structural problems. For example, migraine is a type of primary headache. While primary headaches may cause significant daily pain and disability, they are not dangerous.

Secondary Headache : Secondary headaches are caused by an underlying disease, like an infection, head injury, vascular disorders, brain bleed or tumors. Secondary headaches can be harmless or dangerous. Certain "red flags" or warning signs indicate a secondary headache may be dangerous.

Type of Primary  Headache

Out  of all headaches, 90% are primary headache.Some of these are described below :

Tension Headache : This type of headache begins at the back of neck and spreads upward or it begins at the forehead and goes down to neck.It usually presents with non-pulsing “bandlike” pressure on both sides of the head.The pain is usually moderate and may be related to stress, anxiety, depression,

Migraine headache : This headache usually involves severe throbbing pain associated with nausea, photophobia, phonophobia. It usually begins on one side of head and may spread to both sides.It may occur with aura or without aura i.e. there may be a warning signal before the headache begins.

Primary Cough Headache : It starts suddenly after coughing or sneezing or straining.

Primary Sex Headache : It is dull,bilateral headache that starts during the act of sex and increases till orgasm.They are thought to be due to be due to lower pressure in the head during sex.this headache needs to be differentiated from headace due to sub arachnoid haemorrhage as both begin during sexual activity.

Migraines may be triggered by foods such as chocolate, certain cheeses, or monosodium glutamate (MSG). Caffeine withdrawal, lack of sleep, and alcohol may also be triggers.It is often severe enough to hamper daily activities of the patient . There are episodes of migraine which last from a few hours to days.They are
Cluster Headache :Cluster headaches are very severe type of headaches which are also called "Suicidal Headache".There are short episodes of severe pain,usually one sided i.e.pain around one eye with tears, congestion and red eye.The episode of pain comes at the same time everyday for some days or weeks and it may then disappear for many months.
Cluster headaches are mostly found in heavy smokers.

Primary exertional Headache : THese are throbbing, pulsatile pain in head which starts during or after exercise.It may last from 5 min to 24 hours.Their mechanisim is unknwn but they can be prevented by doing les sternuous exercises.

Trigeminal Neuralgia : It is a type of headache that is due to disease of trigeminal Nerve which suplies face.hence, the pain is present in head and face.

Hypnic Headache : Moderate to severe headache that start a few hours after falling asleep And lasts for 15 - 30 min.Usually older women suffer fom this type of headache.

Types of secondary Headache : Secondary headache is secondary to a condition known to cause headache,hence it is more dangerous than primary headache although it is less common.


It is inflammation of the membrane covering the brain called meninges.
INTRACRANIAL HAEMORRHAGE: bleeding inside the brain which causes headache.
Subarachnoid haemorrhage:Diseases of head and neck like pain due to swelling in the muscles of neck.
Pain due to medicine overuse: It may occur in those using excessive painkillers for headaches, paradoxically causing worsening headaches.
BRAIN TUMOUR: A tumor that is growing in the brain can press against nerve tissue and pain-sensitive blood vessel walls, disrupting communication between the brain and the nerves or restricting the supply of blood to the brain. Thus, causing headaches.
Disorders of blood vessels in the brain, including stroke several disorders associated with blood vessel formation and activity can cause headache. Most notable among these conditions is stroke. Headache itself can cause stroke or accompany a series of blood vessel disorders that can cause a stroke.
Acute closed angle glaucoma (increased pressure in the eyeball): headache that starts with eye pain, blurry vision, associated with nausea and vomiting. On physical exam, the person will have a red eye and a fixed, mid dilated pupil. Differentiating the different varieties if there is pain in head is very important as the headache can be simple and non-malignant or it may have an underlying disease.
Most headaches can be diagnosed by the clinical history alone. If the symptoms described by the person sound dangerous, further testing is required.

The first step to diagnose a headache is to know if it is old or new or an old headache with new symptoms.
The American Headache Society recommends using "SSNOOP", a mnemonic to remember the red flags for identifying a secondary headache

Systemic symptoms (fever or weight loss)
Systemic disease (HIV infection, malignancy)
Neurologic symptoms or signs
Onset sudden (thunderclap headache)
Onset after age 40 years
Previous headache history (first, worst, or different headache)
The tests for diagnosis of headache are performed according to the accompanying symptoms. Various diagnostic modalities are listed below :
