Menopause or climacteric is usually the natural physiological process in the life of a woman in which natural cessation of menstruation occurs, this is accompanied by regression of ovarian functions and females can no longer get pregnant. Menopause can also occur due to physiological removal of ovaries or chemotherapy. Menopause usually occurs between 45 years and 55 years. Menopause is said to have occurred when a female does not have monthly bleeding for twelve months continuously. It should be remembered that menopause is not a disease but a natural physiological process which is accompanied by symptoms like irritability and hot flushes etc. due to hormonal changes.

Signs and Symptoms of Menopause:

The clinical presentation of menopause varies in different females.

  • Mood Changes- Depression, irritability and mood swings are often reported by menopausal women.
  • Lack of concentration.
  • Hot Flashes- These may include cold flushes or night sweats.
  • Headaches- There may be simple headache or migraine.
  • Rapid Heart Beat
  • Irregular Uterine Bleeding- The period may be shorter or longer, lighter or heavier, with more or less time in between.
  • Vaginal atrophy and vaginal dryness
  • Painful sexual intercourse
  • Decreased libido
  • Urinary frequency and urgency
  • Urinary Incontinence- This is a rare symptom of menopause and is usually associated with obstetric events.
  • Increased susceptibility to vaginal and urinary tract infections.
  • Sleep disturbances- Some females may suffer from insomnia while others may have poor sleep or sleepiness during the daytime.
  • Skin and hair changes- Loss of fatty tissue and collagen makes skin drier and thinner.
  • Joint pain and muscle pain.
  • Osteoporosis- There is increased risk of Osteoarthritis in postmenopausal females.
  • Atherosclerosis- Like OA, there is an increased risk of atherosclerosis also in postmenopausal women.

How does Menopause Occur?

Menopause is a part of natural ageing. At birth females have eggs in the form of follicles which are released from ovary every month as a part of menstrual cycle. This happens under the control of female hormones FSH and LH. In pre-menopausal and peri menopausal period the menstrual cycles become irregular and amount of blood increases or decreases. Finally the regular blood shedding ceases. This happens because of a decrease in the FSH and LH hormones due to which the ovaries stop ovulation permanently leading to menopause. Once, a female does not have menstruation for twelve months, she lose the power of giving birth. Along with this, due to hormonal changes females have one or more symptoms described above.

Is menopause a disease? 

No, natural menopause is not a disease. It is a part of normal physiology of females. The symptoms of menopause are due to hormonal changes. It is because of this every female presents with different set of symptoms and needs individualized management and treatment.
